About Your Visit

What should you bring to your appointment at Iowa City Thoracic & Vascular

We want to answer as many of your questions at your visit.

Please prepare for this by bringing:

• Health Insurance Card/Information
• If needed bring a friend or family member to take notes/help with questions
• A list of all medications you are taking
• A list of your questions /concerns
• A copy of your XRAYS / CT scans (not the reports - get the actual films)
• Copies of records from doctors related to your visit with us
• Completed FORMS below

Office Policies

Billing & Insurance

• Please bring your insurance card to your appointment.
• Your insurance co pay is due at the time of your visit.
• Our office will file your insurance claim for you.
• Your billing statement will be mailed to you after the insurance has paid your claim.
